Brenda Shaeffer, PT, DPT is not an insurance provider. We do not bill directly to insurance companies. Payment is expected at the time of service. We accept personal checks, cash, debit/credit cards (no AmEx).
Before your appointment, we recommend you contact your insurance company to see if your plan offers out-of-network benefits. You may be eligible for reimbursement if this is the case. While all insurance plans are different, we find that most plans do not cover 100% of the visit. We recommend that you expect to be fully financially responsible for all treatments under Brenda’s care.
If you plan to submit a claim for reimbursement, we will provide a visit statement electronically upon request. The patient is fully responsible for submitting this claim. Brenda Shaeffer, PT, DPT will not submit to any insurance company, even if your plan offers out-of-network benefits.
Workmen’s compensation and PIP cases will be accepted on a case-by-case basis only. All cases must be approved before scheduling. It is the patient’s responsibility to provide the office with any information regarding worker’s comp/PIP cases.
We require 24 hours for all cancellations. Please expect a full visit charge for any missed appointments.
